Funeral / Memorial Service & Officiant Request Form

Please use this form to let us know about the passing of your loved one, and how we can help host a service that honors their memory and creates space for you and your family to grieve, comfort one another, and celebrate their life. We are so very sorry for your loss, and our first response to this request, once you press submit, will be to pray that God will meet and your family in your time of need with provision, healing, comfort, and peace. When the waves of grief wash over you, and when the days seem overwhelming, remember that “His mercies are new every morning… He is near the broken-hearted… He keeps a record of our tears… He draws near to those who draw near to Him” and that in Christ, we can “approach God’s throne of grace with confidence, so that we may receive mercy and find grace to help us in our time of need.” (Lamentations 3:22-23, Psalm 34:18, Psalm 56:8, James 4:8, Hebrews 4:16)

Note: Due to the many events, small groups, services, and studies being hosted at Grace, we can only consider requests from those who are current and active members of the church. Our first step after receiving a request is to check the database to verify that the applicant or the deceased have a recent and ongoing connection to Grace Fellowship.

If the service is being held off-site, simply enter the date and time in the first spot, leaving the alternate date and time blank. If being hosted at Grace Fellowship, please provide both a preferred and alternate date.
Note: We cannot guarantee that your preferred pastor is available, but we will do our best to accommodate these requests.

If a funeral home or pastor from another church is helping to coordinate the service, who is the primary contact there?

Note: The default Sunday setup in the East Auditorium seats around 250 guests in four sections, with an aisle on the left, center, and right. Due to the number of groups and events that meet in the building each week, we are not able to provide a custom setup of this room, but rather, can only offer it as-is. With that said, we are happy to help provide additional resources as we are able, to allow families to decorate, remember, and honor their loved ones, and to provide space for friends and family members to visit and connect. A list of those resources is provided below. If you have additional questions or needs, don’t hesitate to ask. Once you submit this form, our Care Pastor will follow up to help coordinate the details. We know it’s a difficult time, and our desire is to help carry a burden and not add more to your list. Even so, these things do take planning, and we appreciate your patience and help.

Note: This is limited to one instrument and two singers.
